My weight management journey.

Julia K. was the first patient that started our Weight Management program at Miami Primary. Approximately nine months ago, a patient presented to our medical office seeking assistance with weight loss. At that time, she was classified as obese, with a weight of 217 pounds and a BMI of 34. Despite having attempted various diets without success, the patient was determined to address her weight-related concerns.

Under the care and guidance of Dr. Olga, the patient enrolled in the Miami Primary Weight Loss program, a comprehensive approach designed to facilitate significant and sustainable weight loss. The program incorporated a combination of factors, including a tailored diet plan, special exercises, and the administration of semaglutide medication. Dr. Olga closely supervised the patient throughout the entire process.

After nine months of dedicated efforts, the patient experienced remarkable progress. Her BMI reduced from 34 to 27, indicative of a significant shift toward a healthier weight range. We've been on quite a journey together, haven't we? Can you share with us your experience, starting from the day we began your weight loss treatment? 

Julia K.: Good afternoon, Dr. Olga. Indeed, it's been quite a transformative experience. When we began, my weight had been a persistent challenge, affecting my health and well-being. You introduced me to different medications, starting with Ozempic, as a part of a comprehensive treatment plan. 

Dr. Olga: Can you tell me about your initial experiences with Ozempic? 

Julia K.: Certainly. When we started with Ozempic about 8 months ago, I was a bit hesitant, but your guidance and support made the process more manageable. The GLP-1 receptor agonist helped me control my appetite, making it easier to adopt healthier dietary habits. Gradually, the weight started coming off. 

Dr. Olga: And as we progressed, we introduced Mounjaro into your treatment plan. How did that contribute to your weight loss journey? 

Julia K.: Mounjaro came into play about three months into our journey. It was another piece of the puzzle that you thought would aid in sustained weight loss. The combination of medications, coupled with the lifestyle changes we discussed, seemed to work synergistically. I noticed a boost in my metabolism, and the positive results continued. 

Dr. Olga: Along the way, we encountered some challenges and side effects. How did we navigate through those? 

Julia K.: Like any treatment plan, there were hurdles. I experienced mild nausea with Ozempic initially, but you reassured me that it would likely subside, and it did. Mounjaro brought about some gastrointestinal issues, but your adjustments to the treatment plan and constant communication made the process much smoother.

According to recent studies, it's often recommended to begin with 0.25 mg once a week for the first four weeks, then gradually increase to 0.5 mg for the next four weeks before reaching the maintenance dose of 1 mg. 

Julia K.: Yes, that's correct. I remember that we followed that dosage plan precisely.  Dr. Olga: It's essential to adhere to these guidelines to minimize side effects and ensure a smooth transition. Now, moving forward, how did you find adjusting to these doses, and did you notice any significant changes during this phase? 

Julia K.: Initially, I did experience some mild nausea, as you mentioned, but it did subside after the first few weeks. The gradual increase in the dosage allowed my body to adjust, and I began to see positive changes in my appetite and weight. 

Dr. Olga: Excellent. Now, transitioning to Mounjaro, we integrated it into your treatment plan around the three-month mark. The dosing and regimen for Mounjaro were also tailored to your needs based on the available research. 

Julia K.: Right, we discussed starting with a lower dose and then adjusting as needed. It was comforting to know that we were following evidence-based approaches. 

Dr. Olga: Evidence-based practices are crucial for the success of any treatment plan. Can you share your experiences as we introduced Mounjaro, and how did it align with your overall journey? 

Julia K.: The introduction of Mounjaro brought about some gastrointestinal issues initially, but your guidance and adjustments to the treatment plan helped mitigate those effects. It felt like a complementary addition to the progress we had already made with Ozempic.

Julia K.: Certainly, Dr. Olga. Alongside the medications, we worked on a balanced diet and exercise plan. You recommended a diet rich in lean proteins, whole grains, fruits, and vegetables – a plan that aligns with current dietary guidelines for weight management. 

Dr. Olga: Yes, indeed. Incorporating nutrient-dense foods is crucial for overall health and sustained weight loss. How did you find adjusting to these dietary changes? 

Julia K.: It was a bit challenging initially, but your guidance helped me make gradual adjustments. I found that the medications, especially Ozempic, played a significant role in curbing my appetite and making it easier to stick to the recommended diet. 

Dr. Olga: That's great to hear. Now, let's talk about the exercise component. What type of physical activities did you incorporate into your routine, and how frequently were you engaging in them? 

Julia K.: We discussed a mix of aerobic exercises and strength training. I started with brisk walking and gradually progressed to more intensive workouts. I aimed for at least 150 minutes of moderate-intensity aerobic activity per week, in addition to strength training exercises at least two days a week. 

Dr. Olga: It's wonderful that you were able to integrate both cardiovascular and strength training exercises into your routine. Physical activity is a vital component of a comprehensive weight loss plan. 

Julia K.: Absolutely. The combination of the medications, dietary changes, and regular exercise seemed to work synergistically. I felt more energized and n oticed positive changes not only in my weight but also in my overall well-being.   Dr. Olga: As you know, there is a new FDA approved weight loss drug on the market – Zepbound. In the latest studies, researchers reported that over 72 weeks, participants who took the highest dose of Zepbound lost an average of 48 lbs, and those who took the lowest dose lost an average of 34 lbs. Would you like to try Zepbound (tirzepatide) after we are done with Mounjare? 

Julia K.: Definitely! I also read that it has a positive effect on high blood pressure and high cholesterol. 

Dr. Olga: And now, looking at your overall health, how do you feel since we started this weight loss journey together? 

Julia K.: My overall health has undergone a remarkable transformation. I've not only lost a significant amount of weight, but my energy levels have soared. My blood pressure and cholesterol have improved, and I feel more confident and capable of maintaining a healthy lifestyle.
